Job summary

Valor Healthcare is seeking a passionate Licensed Practical Nurse to join our Community Based Outpatient Clinic (CBOC) in Cranberry Township. The role offers a 4-day workweek, competitive salary, excellent benefits, generous time off, and a weekday schedule.

You will provide prescribed medical treatment and collaborate with the PACT team to develop patient care plans, while maintaining high standards of care and patient service.

Qualifications

• Diploma or certificate of an accredited practical nursing program.
• Minimum one year of LPN/LVN experience in primary care or ambulatory care.
• Must be credentialed and in good standing with VA or state licensure.
• Current BLS certification and ACLS as required by VA contract.
• Proficient in English and strong computer/EMR skills.

Responsibilities

• Provide prescribed medical treatment and personal care to patients.
• Collaborate with PACT team to develop patient care plans.
• Administer medications and document in EMR.
• Perform nursing assessments and health education for patients.
• Maintain confidentiality and comply with training requirements.
