Job summary

Palms-Home-Care is expanding and seeking a dedicated Home Health LPN to join our compassionate team. You will provide patient-centered care in the home, with emphasis on safety, education, and collaboration with physicians and therapists.

The role requires an active LPN license, strong nursing skills, and a patient-focused approach to improve quality of life for clients in their homes.

Qualifications

• Valid LPN license in good standing.
• Experience as a licensed practical nurse, preferably in home health.
• Excellent knowledge of nursing care methods and procedures.
• Knowledge of health and safety guidelines and procedures and willingness to follow them.
• Team player with excellent communication and interpersonal skills.
• Responsible, compassionate and organized with strong multitasking skills.

• Chart observations of the patient’s condition at each visit.
• Administer Physician-prescribed medication.
• Dress or redress wounds and monitor the healing progress.
• Provide education to patients and families on proper home health care procedures and strategies.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Recommend tools or devices that may improve the quality of life for the patient.

Qualifications
