Job summary

Palms-Home-Care is seeking a dedicated Home Health LPN to provide patient-centered care in home settings across the Sarasota/Bradenton area. You will document observations, administer medications, and support wound care, education, and care coordination with a multidisciplinary team.

The role requires a valid LPN license and Home Health experience, with a strong emphasis on safety, compassion, and excellent communication. Flexible scheduling and competitive benefits are offered.

Responsibilities

• Chart observations of the patient's condition at each visit.
• Administer Physician-prescribed medication.
• Dress or redress wounds and monitor the healing progress.
• Provide education to patients and families on proper home health care procedures and strategies.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient's care plan.
• Recommend tools or devices that may improve the quality of life for the patient.
