Pinnacle Home Care in Florida and beyond is seeking a Licensed Practical Nurse (LPN) for full-time field assignments. You will administer skilled nursing care and support patient goals under RN supervision, with a focus on home-based, individualized care that improves outcomes and patient satisfaction.

Ideal candidates hold a current LPN license, BLS, and at least one year of home health or clinical nursing experience.

Qualificações

• Must have a current LPN or LVN license (state-specific).
• Graduate of an accredited nursing program.
• Valid BLS certification.
• Minimum one year of nursing experience (home health or medical/surgical preferred).
• Excellent communication, organizational, and time management skills.
• Must have a valid driver’s license and access to a reliable and insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.

Key Responsibilities

• Complete visit reports promptly and accurately within the electronic medical record system to support high-quality care and regulatory compliance, including all required documentation.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Understand and adhere to the agency’s process for managing and addressing customer complaints.
• Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
• Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.
