Job summary

Palmshomecare is seeking a dedicated Licensed Practical Nurse (LPN) to provide home health care. The role includes charting patient observations during visits, administering physician-prescribed medications, wound care, and patient education to families on proper home care procedures.

You will coordinate with physicians and therapists to align the care plan and may recommend devices to improve quality of life. A team player with strong communication and organizational skills is essential.

Qualifications

• Valid LPN license in good standing.
• Proven experience as a licensed practical nurse, preferably in home health.
• Excellent knowledge of nursing care methods and procedures.
• In-depth knowledge of health and safety guidelines and procedures and willingness to follow them.
• A team player with excellent communication and interpersonal skills.
• Responsible and compassionate.
• Outstanding organizational and multi-tasking skills.
• Patient with excellent problem-solving skills.
