Shanadoa Home Health Inc. in Oklahoma is seeking an LPN to join the Southwest Territory for full-time home health visits. You will provide skilled nursing care in patients' homes, work with an RN partner, and help families manage care plans in familiar, comfortable settings.

You will conduct nursing visits, wound care, med management, vital signs, patient education, and timely documentation with your RN partner.

Qualifications

• Active Oklahoma LPN license required.
• Home health nursing experience preferred.
• Strong patient communication and documentation skills.

What You'll Do

• Provide skilled nursing visits under the direction of your RN partner and the plan of care
• Perform wound care, medication management, vital signs, and disease management
• Teach patients and families how to manage their health at home
• Report changes in condition to your RN partner and the care team
• Document accurately and on time

What You'll Need

• Active Oklahoma LPN license
• Valid driver's license and a clean driving record
• Home health experience preferred, but if you have solid clinical skills and a heart for patients, we'll teach you the rest
