Job summary

Adams Farm Living is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Jamestown, NC. The role focuses on providing direct patient care to geriatric residents in a long-term care setting, administering medications, monitoring health status, and supporting families and the care team.

Qualified candidates will have an accredited LPN program graduate, an active LPN/LVN license in North Carolina, and CPR/BLS certification.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted LPN/LVN license in the state of practice.
• Experience providing direct patient care to geriatric and long-term care populations.
• Experience with electronic health records (EHRs) and clinical documentation.
• Knowledge of infection prevention and control practices.
• Strong assessment, communication, and critical-thinking skills.

Responsibilities

• Record a patient’s medical history accurately.
• Take and record measurements of blood pressure, temperature, heart rate, etc.
• Observe patients under treatment to identify progress and side-effects.
• Monitor patients’ condition including intake/output and charting.
• Assist with daily activities such as eating and bathing.
• Administer medications and physician-ordered therapies safely.
• Provide emotional support and communicate with families as needed.
• Coordinate with the interdisciplinary care team.
