Job summary

Century Care Management Inc. seeks an experienced Licensed Practical Nurse (LPN/LVN) to join our long-term care team. You will record histories, monitor vitals, and support residents with daily activities in a compassionate setting.

You will administer medications, coordinate with the interdisciplinary team, and ensure high-quality patient care with compliant documentation.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted LPN/LVN license in the state of practice.
• Knowledge of long-term care regulations and resident rights.
• Experience providing direct patient care to geriatric/long-term populations.
• Ability to administer medications and therapies safely and accurately.
• Experience with electronic health records and clinical documentation.
• Strong assessment, communication, and critical-thinking skills.
• Ability to supervise and delegate tasks to CNAs in accordance with state regulations.
• Compassionate, resident-focused approach to care.

Responsibilities

• Record a patient’s medical history accurately.
• Take and record measurements of blood pressure, temperature, heart rate, etc.
• Monitor patients’ condition and report changes to the interdisciplinary care team.
• Assist patients in daily activities such as eating, bathing, and dressing.
• Administer injections, prescribed medications, and therapies.
• Collect samples for lab testing as assigned.
• Provide emotional support and communicate with families.
• Maintain infection prevention and control practices.
