Job summary

Diamond Willow Assisted Living in Little Falls, MN is seeking an LPN to lead and support a compassionate care team. You will manage medications, document health needs, and coordinate care with the RN Director while maintaining confidentiality in a resident-focused setting.

The role requires an approved LPN program graduate with Minnesota licensure, and a willingness to cover aide shifts including weekends. This is a full-time on-site position offering a supportive team environment.

Responsibilities

• Medication management and chart auditing
• Perform resident treatments as ordered per care plan
• Observe, tend to and document residents’ health needs
• Communicate with RN Director regarding staff and resident information/concerns
• Review appointments and arrange transportation for residents
• Process MD orders completely and accurately
• Complete incident reports as needed
• Ensure staff perform safe practices
• Conduct tours for potential admits
• Review messages and communicate with staff via R-Task

Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Our facility in Little Falls, MN consists of 2 cottages/suites and up to 26 residents. • Medication management & chart auditing
• Performing resident treatments as ordered per the care plan
• Observing, tending to and documenting residents health needs
• Communicating with RN Director regarding relevant staff and resident information and/or concerns
• Reviewing appointments & setting up transportation for residents
• Processing MD orders completely and accurately
• Completing incident reports as needed
• Ensure the staff is performing safe practices
• Executing tours for potential admits
• Reviewing messages and communicating with staff through R-Task

Qualifications:

• Graduate from approved LPN program
• Possess a valid Minnesota LPN License

Requirements:

• Minimum of 1 year experience
• Leadership ability
• Must be able to occasionally fill in aide shifts when needed, this could include weekends.
• Maintaining confidentiality and professionalism at all times
• Must be able to work as a team member and maintain a positive demeanor
