Job summary

Diamond Willow and Keystone Bluffs Assisted Living in Little Falls, MN seeks an experienced LPN to lead care delivery. The role includes medication management, care plan execution, and coordination with the RN Director. You must be licensed in Minnesota and have at least 1 year of experience in senior care. This is a full-time, on-site position with weekend coverage as needed.

Qualifications

• Graduate from approved LPN program.

Responsibilities

• Medication management and chart auditing.
• Performing resident treatments as ordered per care plan.
• Observation and documentation of residents' health needs.
• Coordinate with RN Director on staff and resident information.
• Review appointments and arrange transportation for residents.
• Process MD orders accurately.
• Complete incident reports as needed.
• Ensure staff perform safe practices.
• Conduct tours for potential admits.
• Communicate with staff through R-Task.

Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Our facility in Little Falls, MN consists of 2 cottages/suites and up to 26 residents. Responsibilities

• Medication management & chart auditing
• Performing resident treatments as ordered per the care plan
• Observing, tending to and documenting residents health needs
• Communicating with RN Director regarding relevant staff and resident information and/or concerns
• Reviewing appointments & setting up transportation for residents
• Processing MD orders completely and accurately
• Completing incident reports as needed
• Ensure the staff is performing safe practices
• Executing tours for potential admits
• Reviewing messages and communicating with staff through R-Task

Qualifications

• Graduate from approved LPN program
• Possess a valid Minnesota LPN License
• Minimum of 1 year experience
