Job summary

Great Lakes Management Co. is seeking an LPN to join our team in Roseville, MN. This role offers a flexible 20-hour weekly schedule with compensation ranging from $30 to $32 per hour.

MN LPN certification is required to ensure quality care for our senior residents. As an LPN in our assisted living communities, you will support resident care plans, coordinate with the care team, and help maintain a safe, nurturing environment.

Qualifications

• Current state LPN license in MN required.
• Minimum of 1 year experience in home health, assisted living or long-term care preferred.
• Strong organizational skills and ability to work with minimal supervision.
• Empathy and passion for elderly care.

Responsibilities

• Ensure residents meet admission criteria and monitor needs.
• Maintain up-to-date care plans per state regulations.
• Monitor and document clinical progress notes and charts.
• Communicate changes to physicians, staff, and families.
• Coordinate resident services with other providers.
• Oversee medications and renewals; coordinate physician follow-up.
• Ensure proper medication administration procedures are followed.
• Record relevant information in the resident log for staff awareness.
• Provide alternating supervisory visits with the RN.
• Collaborate with the care team to ensure safety and quality of care.

LPN R esponsibilities:

• Assures residents maintain admission criteria, monitor , and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident’s needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident’s log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN .
• Works as a team player with all members of the residents’ care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate .
• Works with manager to assure emergency procedures are in place

LPN Qualifications :

• Maintains a current state license as a n LPN ( Licensed Practical Nurse ) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
• Exhibits a caring heart with a passion to care for the elderly
