Job summary

Sunrise Senior Living in the United States is seeking a Wellness Nurse LPN to monitor resident health, oversee medications, and uphold clinical quality across the community.

You will perform assessments, document care plans, collaborate with families and physicians, and support infection control and regulatory standards. A good standing LPN license and at least one year in senior care are required; strong communication and documentation skills are essential.

Qualifications

• Active LPN license in good standing.
• Graduate of approved nursing program.
• 1+ year experience in senior care or clinical setting.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments.
• Perform monthly wellness visits and monitor health trends.
• Follow up on falls, incidents, and hospital returns.
• Transcribe physician orders and ensure accurate MAR/TAR documentation.
• Complete medication audits and oversee medication compliance.
• Partner with families, physicians, and the Resident Care Director.
• Support infection control and regulatory standards.
• Educate and guide care staff on documentation and medication practices.

What We’re Looking For:
