Job summary

Sunrise Senior Living is seeking an experienced LPN to monitor resident health, support medication oversight, and maintain clinical quality across the community.

The role requires an active LPN license, graduation from an approved nursing program, and at least 1 year of senior care or clinical setting experience. You will conduct assessments, translate orders, and collaborate with families and physicians.

Qualifications

• Active LPN license in good standing.
• Graduate of approved nursing program.
• 1+ year experience in senior care or clinical setting.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments.
• Perform monthly wellness visits and monitor health trends.
• Follow up on falls, incidents, and hospital returns.
• Transcribe physician orders and ensure accurate MAR/TAR documentation.
• Complete medication audits and oversee medication compliance.
• Partner with families, physicians, and the Resident Care Director.
• Support infection control and regulatory standards.
• Educate and guide care staff on documentation and medication practices.
• Help prevent decline through proactive assessment, early intervention, and strong clinical oversight.
