Sunrise Senior Living is seeking a Licensed Practical Nurse (LPN) to monitor resident health, oversee medication practices, and maintain clinical quality across the community. You will conduct various assessments, monthly wellness visits, and coordinate with families and physicians to ensure high standards of care.

The role requires active LPN licensure, nursing program graduation, and at least 1 year in senior care or a clinical setting.

• Graduate of approved nursing program.
• 1+ year experience in senior care or clinical setting.
• Strong assessment, documentation, and communication skills.

Responsabilidades

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments.
• Perform monthly wellness visits and monitor health trends.
• Follow up on falls, incidents, and hospital returns.
• Transcribe physician orders and ensure accurate MAR/TAR documentation.
• Complete medication audits and oversight of medication compliance.
• Partner with families, physicians, and the Resident Care Director.
• Support infection control and regulatory standards.
• Educate and guide care staff on documentation and medication practices.
