Job summary

Mosaicms is seeking a Memory Care Wellness Manager (LPN) in Oregon to lead clinical oversight and supervision in our memory care community. You will oversee resident care, assessments, service plans, and progress documentation to ensure high-quality, compliant care.

The role requires an active LPN license, at least 2 years of supervisory healthcare experience, and strong leadership with a resident-centered approach. EMR proficiency and collaboration with the care team are essential.

Qualifications

• Active LPN/LVN license in good standing per state regulations.
• Minimum 2 years of supervisory experience in senior living, assisted living, or healthcare preferred.
• Strong understanding of medication management, care planning, regulatory compliance, and best practices in senior care.
• Excellent leadership, coaching, and conflict-resolution abilities.
• Proficiency with EMRs, MARs, care documentation, and Microsoft Office Suite.

Responsibilities

• Clinical Oversight & Resident Care: Oversee delivery of resident care, assessments, service plans, and progress documentation.
• Documentation: Review updates daily in the community communication log and contribute to service plan reviews.
• Regulatory & Orders: Review physician orders, MARs, and care documentation for accuracy and compliance; ensure medication administration aligns with best practices.
• Escalation: Escalate unresolved clinical issues or medication discrepancies to the RN or Executive Director as needed.
• Medication & Health Systems Management: Oversee the medication delivery system to ensure safety, accuracy, and regulatory compliance.
