Job summary

TMS is seeking a Registered Nurse for long-term care to provide and coordinate nursing care for residents using assessment, planning, implementation and evaluation. You will administer medications, develop and update care plans, and supervise licensed practical nurses and nursing assistants to ensure high-quality care.

The role requires RN licensure in the state, BLS certification, and graduation from an accredited school of nursing.

Qualifications

• Graduate of an accredited school of nursing: Required.
• Prior experience in a long-term care or skilled nursing setting: Preferred.

Responsibilities

• Assess residents on admission and on an ongoing basis, and develop individualized plans of care.
• Administer medications and treatments in accordance with physician orders and facility policy.
• Supervise, delegate to and provide clinical direction for LPNs and Certified Nursing Assistants.
• Evaluate resident response to interventions and revise the plan of care as needed.
• Maintain accurate, timely clinical documentation and support regulatory survey readiness.
• Coordinate care with physicians, therapy, families and interdisciplinary team members.
