Care Med in Ohio is seeking Nurse Practitioners to provide medical services to residents living in long-term care facilities. You will join an interdisciplinary team to deliver high-quality care and improve residents' wellbeing.

You will deliver primary and preventive care, handle acute changes, plan for advanced and chronic care, prescribe medications per Ohio law, order tests, coordinate with nursing home staff, and collaborate with primary care physicians.

Qualifications

• Current, unrestricted NP license in the state of OH.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus

Responsabilités

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.
