Job summary

CareMed Health Solutions in Ohio seeks experienced Nurse Practitioners to deliver primary and preventive care to residents in long-term care facilities. You will provide comprehensive medical services within an interdisciplinary care team and contribute to quality resident outcomes.

Role requires active OH NP license and board certification, with DEA licensure, EMR proficiency, and a background in geriatric or family medicine. This is a full-time in-facility position with competitive benefits.

Qualifications

Current, unrestricted NP license in OH
Board Certification in Nursing Practice
DEA licensure required by OH Law
Clinical background in adult, family or geriatric specialties
Experience in long-term care settings a must
Proficient with EMR and care planning
Ability to manage patients with advanced illness and chronic conditions
Ability to work within an interdisciplinary care team
Psych experience a plus

Responsibilities

Provide primary and preventive care for long-term care patients in skilled nursing facilities
Manage residents with acute changes in condition
Advanced care planning and chronic care management
Prescribe medications, including controlled substances per OH Law
Order, review and follow up on treatments and diagnostic testing
Communicate with nursing home staff to discuss changes in condition and care plans
Collaborate with primary care physician to coordinate patient care
