Myaccessmedicalclinic in Mountain Home, Arkansas is seeking a Long Term Care Advanced Practice Provider (APP) who will deliver high autonomy clinical care in skilled nursing and long term care facilities. The role requires performing assessments, managing conditions, and developing care plans while collaborating with medical teams.

You’ll support quality improvement initiatives and participate in care transitions. Ideal candidates will have an advanced degree, active licensure, and strong clinical judgment.

Qualifications

• Active state licensure and board certification (AANP/ANCC/NCCPA) required.
• 2+ years of clinical experience preferred.
• Experience in Long Term Care, Skilled Nursing Facility, or geriatrics is a plus.

Description

The Long Term Care Advanced Practice Provider (APP) delivers high autonomy clinical care within skilled nursing and long term care facilities, working closely with medical directors, nursing teams, and interdisciplinary partners to improve resident stability and reduce avoidable hospitalizations. Details include:

• Perform comprehensive assessments and manage acute and chronic conditions common in LTC populations.
• Develop and adjust individualized care plans in collaboration with facility teams.
• Manage high risk LTC needs including infections, wounds, CHF/COPD, dementia, and polypharmacy risks.
• Serve as a consistent clinical presence supporting workflows, new admissions, and care transitions
• Participate in interdisciplinary care planning, education, and quality improvement initiatives.
• Support regulatory readiness, documentation accuracy, and survey/audit preparation.
• Utilize strong independent clinical judgment and elevate care appropriately.
