Job summary

SCO Family of Service in New York is hiring an LMSW / Client Care Coordinator for NYC shelters in the Bronx. You will perform biopsychosocial assessments and connect families to community services, working with a multi-disciplinary team.

Qualifications

• MSW degree required; LMSW licensure within 3 months of hire.
• Experience in child welfare, mental health, family-centered practice.
• Strong written and verbal communication skills.
• Proficiency in Spanish or French is a plus.

Responsibilities

• Completes biopsychosocial assessments.
• Facilitates linkages for families to community-based services and coordinates services from multiple city agencies.
• Maintains a multi-disciplinary team approach to service provision.
• Provides information and training to other team members around clients’ psychosocial needs to ensure positive outcomes.
• Prioritize and visit high-risk families in shelter to coordinate safety planning.
• Provide direct counseling services and/or outreach; make referrals as needed.
• Complete all documentation in a timely manner.
