Qualifications

• Must be LMSW licensed in NY or obtain licensure within three months of hire.
• Master’s degree in social work from an accredited school.
• Experience in diverse cultures and homeless/housing services preferred.
• Spanish and/or French language skills a plus.

Responsibilities

• Complete a comprehensive bio-psychosocial assessment with each client to understand strengths and service needs.
• Provide clear written reports that capture assessment findings and recommendations.
• Assist homeless single men as they navigate multiple systems and cope with stressors of homelessness.
• Provide outreach, crisis intervention, risk assessment, safety planning, and psycho-education.
• Coordinate safety planning and unit visits for high-risk individuals in-shelter.
• Collaborate with ACS and prevention services and advocate for the men.
• Make referrals and linkages between mental health providers, government agencies, and community services.
• Improve access to mental health services through in-person or telehealth options.
• Deliver enhanced mental health services: assessment, on-site services, and care coordination.
• Utilize telehealth services to promote access and coordinate care.
