Job summary
Homes for the Homeless in the Bronx is seeking a Client Care Coordinator to provide mental health and related services to homeless families, perform intake assessments, and collaborate with case management to connect families with community resources for lasting success. The role emphasizes trauma-informed, family-centered care, shelter support, and diligent data documentation.
A master’s degree in social work or mental health counseling and NY LMSW/LMHC licensure are required; bilingual skills
Qualifications
- Must have a master’s degree in social work or Mental Health Counseling.
- Applicants must hold an LMSW or LMHC in New York.
- Familiarity with substance use disorders, domestic violence, trauma and mental health conditions.
Responsibilities
- Complete a comprehensive bio-psychosocial assessment with each family.
- Monitor, lead and coordinate social services and counseling support.
- Ensure focus on trauma-informed and family-centered care.
- Monitor and document client progress toward service plan goals.
- Refer families for counseling or other services as needed.
- Support the Family Services department and assist with trainings.
