Job summary

Children’s Rescue Fund in the Bronx, NY seeks a Client Care Coordinator to oversee mental health and related services for homeless individuals, conducting biopsychosocial assessments and coordinating with case management to set goals and link clients to community resources.

You will deliver on-site services, crisis intervention, and referrals, work in a multidisciplinary team, and support clients toward independent living, with required LMSW licensure and Spanish or French a plus.

Qualifications

• Must be LMSW in NY at hire or within 3 months
• Master’s degree in social work from an accredited school
• At least 1 year in social services, counseling, crisis intervention or related fields preferred
• Knowledge of diverse cultures and ethnicities
• Strengths-based, solution-focused, family-centered practice
• Experience facilitating groups for adults
• Spanish and/or French proficiency a plus
• Excellent written and verbal communication skills
• Excellent interpersonal skills and MS Office literacy

Responsibilities

• Complete a comprehensive bio-psychosocial assessment for each client
• Provide clear written reports of assessments and recommendations
• Assist homeless men as they navigate multiple systems and cope with stress
• Provide outreach, crisis intervention, risk assessment, safety planning, and psycho-education
• Prioritize unit visits to high-risk individuals for safety planning
• Prepare men to accept services
• Collaborate with ACS and prevention agencies and advocate for men
• Make referrals and facilitate linkages to mental health providers and community services
• Serve as liaison with government and community groups
• Improve access to mental health services via in-person or telehealth
• Work in a multidisciplinary team to enhance engagement and care
• Deliver enhanced mental health services and coordinate clinical care
• Promote in-person or telehealth usage with Express Care and telehealth services
• Utilize DHS Referral Basics to schedule appointments and arrange transportation
• Follow up on referrals and reschedule as needed
• Use motivational interviewing to re-engage clients
