• Credentials from multiple states required at hire; additional credentials post-hire.

Responsibilities

• Collaborate with health care team to coordinate cost-effective services aligned with care plan.
• Conduct psychosocial assessments to evaluate goals, support, resources, and functioning.
• Develop and monitor a plan of care with nurse case manager to optimize functioning and quality of life.
• Identify and advocate for community resources; ensure timely, cost-efficient implementation.
• Document case management activities per standards; monitor program outcomes and improvements.
• Facilitate access to local, state, and federal programs (Medicaid, Medicare) as needed.
• Provide supportive counseling and education to members, families, and caregivers; address end-of-life issues.
• Maintain knowledge of laws and NASW Ethics; coordinate family meetings; assist with guardianship paperwork.
