Job summary

Kaiser Permanente is seeking a licensed social worker/case manager to coordinate care for a defined population within the health system. You will collaborate with nurses and other clinicians to arrange cost-effective services aligned with the member's plan of care, promoting independence and quality of life.

The role requires MSW, at least one year of clinical social work experience in a healthcare setting, and multi-state licensure within 6 months of hire.

Qualifications

• Minimum one year of clinical social work experience in a health care setting.
• MSW degree required.
• Licensure in multiple states within specified timelines after hire.

Responsibilities

• Coordinate with health care team to facilitate appropriate, cost-effective services.
• Conduct psychosocial assessments addressing goals, support, health status, and resources.
• Develop and monitor a plan of care to promote functioning and quality of life.
• Identify and connect members with local, state, and federal programs.
• Document case management activities in accordance with standards.
• Analyze outcomes to improve program quality and cost effectiveness.
• Assist with guardianship paperwork and access to court proceedings when needed.
• Provide counseling and education to members, families, and caregivers.
