Job summary

Cleveland Clinic Weston Hospital is seeking a Transplant Coordinator to oversee liver transplant activities in the Weston campus as part of the Florida region. You will support care planning, review medical information, and coordinate transplants to ensure exceptional patient experiences.

The role includes coordinating with physicians, staff, and insurance for transplant processes. Responsibilities include preparing cases for the Selection Committee, placing patients on the national waiting

Qualifications

• Two years of RN experience.
• One year of acute care disease-related hospital experience.
• BLS certification through the American Heart Association or American Red Cross.

Responsibilities

• Oversee and manage all liver transplant patient activities.
• Assist with care plans, assemble test results, and coordinate transplants.
• Coordinate scheduling with administrative staff for follow-up appointments.
• Review medical information from referring physicians and initiate communication.
• Coordinate with multiple disciplines and obtain follow-up information.
• Prepare and present cases to the Selection Committee.
• Place patients on the national waiting list for transplantation.
• Coordinate ongoing care of inpatients and outpatients waiting for transplant.
