Qualifications

• BLS certification from the American Heart Association (AHA) or American Red Cross
• Two years of RN experience
• One year of acute care disease-related hospital experience

Responsibilities

• Oversee and manage Kidney transplant patient activities at Weston Hospital.
• Assist fellow caregivers in creating care plans and reviewing medical information.
• Coordinate scheduling with the administrative coordinator for follow-up appointments.
• Review information from referring physicians and initiate appropriate communication.
• Coordinate with other disciplines and obtain follow-up information consultations.
• Coordinate with hospital unit personnel during transplant patient and donor discharge.
• Assist in obtaining medications and supplies according to patient insurance coverage.
