Job summary

Mayo Clinic is seeking a Transplant Coordinator Nurse to oversee coordination of all care aspects for transplant patients. You will work with a multidisciplinary team to assess, plan, educate and lead care across the continuum.

You will coordinate patient education and family support, participate in quality improvement and adhere to standards from ANA, UNOS and FACT. This role requires RN licensure, BSN from ACEN/CCNE program, and eligibility for TN sponsorship.

Qualifications

• Graduate of a BSN program accredited by ACEN or CCNE.
• Minimum 1 year of clinical RN experience required, 3 years preferred.
• RN license by applicable state requirements and current BLS.
• Must obtain CCTC or CCTN within three years of hire; may require OC N/CPHON or BMTCN for bone marrow transplant.

Responsibilities

• Coordinate all aspects of care for transplant patients across the care continuum.
• Assess data and plan in collaboration with patient, caregiver, and multidisciplinary team.
• Provide patient and family education on a consistent basis; lead quality improvement activities.
