Job summary

Pathways Healthcare, a physician-led home health organization, seeks an LICSW social worker to deliver in-home psychosocial assessments and care planning in Londonderry, NH. You will collaborate with nurses and the interdisciplinary team to support patients and families at home.

The role requires an MSW, LICSW in good standing, and at least 1 year of health care experience, with CPR/BLS certification and a valid driver’s license. Per visit pay, mileage reimbursement, and 401(k) match are offered.

Qualifications

• Holds a master's degree from a school of social work accredited by the CSWE.
• Holds a valid LICSW license in good standing.
• Minimum of one year experience in health care, preferably home health.
• Demonstrates excellent verbal and written communication along with strong organizational abilities to manage caseloads.
• Maintain current BLS CPR certification.

Responsibilities

• Conduct thorough assessments of patients' psychosocial status and communicate findings to the RN for holistic care.
• Carry out social evaluations and develop intervention plans to enhance patient experience and outcomes.
• Maintain accurate and up-to-date clinical records for all referred patients.
• Provide information and referral services to patients, families, and caregivers regarding practical needs.
• Act as liaison between patients/families and community agencies to leverage resources.
• Collaborate with the interdisciplinary team to develop comprehensive plans of care and participate in case conferences.
• Assist physicians and team members in understanding social factors impacting health.
• Participate in discharge planning to ensure appropriate post-care support.
