Job summary

Pathways Healthcare is seeking a licensed independent clinical social worker (LICSW) for per diem home health. You will conduct psychosocial assessments, coordinate with the interdisciplinary team, and develop care plans to support patients in York and surrounding areas.

Requirements include a CSWE-accredited MSW, LICSW in good standing, at least 1 year in health care, strong communication, BLS CPR, and a valid driver’s license with a reliable vehicle.

Qualifications

• Holds a master’s degree from a CSWE-accredited school of social work.
• Holds a valid LICSW license in good standing.
• Minimum of 1 year experience in health care, preferably home health.
• Excellent verbal and written communication skills and strong organizational abilities.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and reliable vehicle.

Responsibilities

• Conduct thorough psychosocial assessments of patients’ status in relation to illness and environment.
• Communicate assessment findings to the interdisciplinary home health team for holistic care.
• Carry out social evaluations and develop intervention plans based on findings.
• Maintain accurate clinical records for patients referred to social work.
• Provide information and referral services to patients, families and caregivers.
• Act as liaison between patients/families and community agencies to leverage resources.
• Work with the team to develop care plans and participate in case conferences.
• Assist physicians and team in understanding social factors impacting health problems.
• Participate in discharge planning to ensure appropriate post-care support.
