Job summary

Pathways Healthcare, Llc is seeking a licensed LICSW to lead psychosocial assessments for patients in a home health setting. You will work with a multidisciplinary team to ensure holistic, patient-centered care and effective discharge planning.

The ideal candidate holds a master's in social work and an active LICSW license, with at least 1 year in healthcare, preferably home health. A valid driver’s license and reliable vehicle are required; BLS CPR certification is expected.

Qualifications

• Master's degree in social work from CSWE-accredited program.
• Active LICSW license in good standing.

Responsibilities

• Assess patients' psychosocial status and relation to illness and environment.
• Communicate findings to the home health team for holistic care.
• Develop intervention plans based on assessments to improve patient outcomes.
• Maintain accurate clinical records for referred patients.
• Provide information and referrals to patients and caregivers.
