Job summary

BrightSpring Health Services is seeking a Licensed Clinical Social Worker (LCSW) for Neuro Community-Based Care in Manchester, NH. This role emphasizes real-world psychosocial support delivered in home and community settings, guiding patients and families through adjustment and recovery.

Qualifications

• Bachelor's degree in social work required; Master's degree preferred.
• MSW from a CSWE-accredited program is highly desired.
• CPR certification required and health clearance as per policy.

Responsibilities

• Provides direction, supervision and evaluation of medical social services in the home.
• Instructs, treats, observes and evaluates patients with social and emotional problems affecting health.
• Assists physicians and health team in understanding social factors related to health issues.
• Participates in discharge planning and care coordination processes.
• Maintains HIPAA privacy and complies with company policies and standards.
