Job summary

Qualifications

Responsibilities

• Conduct psychosocial assessments and communicate findings to the nursing team.
• Develop intervention plans based on assessments.
• Maintain accurate clinical records for referred patients.
• Provide information and referrals to patients, families, and caregivers.
• Act as a liaison with patients, families, and community agencies.
• Collaborate with healthcare professionals to support patient care and discharge planning.
