Job summary

Pathways Healthcare, Llc is a physician- and nurse practitioner-led home health organization delivering comprehensive medical rehabilitation in patients' homes. The role focuses on social work within an interdisciplinary team to enhance patient outcomes and ensure smooth transitions.

The social worker will conduct psychosocial assessments, develop intervention plans, maintain records, and connect patients and families with community resources.

Qualifications

• Master's degree in social work.
• Licensed Independent Clinical Social Worker (LICSW) in good standing.
• Minimum 1 year experience in health care, preferably home health.
• Excellent communication and organizational abilities for caseloads.
• Maintain current BLS CPR certification.
• Valid driver's license and own reliable, insured vehicle.

Responsibilities

• Assess patients' psychosocial status and communicate findings to the RN for holistic care.
• Develop social intervention plans and contribute to patient care outcomes.
• Maintain accurate clinical records for patients referred to social work.
• Provide information and referrals to patients and families on practical needs.
• Act as liaison between patients/families and community resources.
• Collaborate with the interdisciplinary team for care plans and case conferences.
• Assist physicians and team in understanding social/emotional factors affecting health.
• Participate in discharge planning to ensure post-care support.

Responsibilities:

• Conduct thorough assessments of patients' psychosocial status in relation to their illness and environment. Communicate assessment findings to the registered nurse to ensure a holistic approach to patient care.
• Carry out social evaluations and develop effective intervention plans based on assessment findings. These interventions will contribute to enhancing the overall patient experience and care outcomes.
• Maintain accurate and up-to-date clinical records for all patients referred to social work, ensuring continuity and quality of care.
• Provide valuable information and referral services to patients, families, and caregivers concerning practical and environmental needs, fostering a supportive and informed healthcare environment.
• Act as a liaison between patients or families/caregivers and community agencies, building collaborative relationships to leverage additional resources for the benefit of patients and organization personnel.
• Work closely with the interdisciplinary team to develop comprehensive plans of care and participate in case conferences as required, facilitating seamless patient transitions and continuity of care.
• Assist physicians and other team members in understanding significant social and emotional factors that impact patients' health problems, contributing to a patient-centered care approach.
• Participate in discharge planning, ensuring patients receive appropriate support and resources after leaving the care setting.

Qualifications:

• Holds a master's degree from a school of social work accredited by the Council on Social Work Education, showcasing a strong educational foundation for this role.
• Holds a valid license as an LICSW in good standing, demonstrating expertise and adherence to professional standards.
• Possesses a minimum of one (1) years' experience in health care, with preference given to candidates experienced in home health care.
• Demonstrates excellent verbal and written communication skills, along with strong organizational abilities to manage caseloads effectively.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
