Job summary

Elara Caring in Mentor, OH is seeking a Licensed Vocational / Practical Nurse to join our home health team. You will follow care plans, monitor progress, and support patients and families in managing health at home. This role supports the RN Case Manager, documents med regimens, communicates with physicians, and helps prevent hospitalizations.

A valid LVN/LPN license, 1 year of clinical experience, and reliable transportation are required. Hospice/home health experience is preferred.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Valid driver's license or reliable transportation to travel between patient homes.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
