Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse in Lafayette, IN. The LPN will follow patient care plans, monitor progress, and collaborate with the RN Case Manager to adjust plans as patient needs change.

You will document medications, communicate with physicians and families, and help prevent hospitalizations through proactive, compassionate care delivering positive outcomes for patients in home health and hospice settings.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
