Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to provide in-home care across Lorain County, Ohio. You will follow patient care plans, monitor progress, and educate patients and families on self-care, while coordinating with the RN Case Manager to adapt plans as patient needs evolve.

The ideal candidate holds an active LPN license, has a nursing certificate or degree, and at least one year in clinical care. A valid driver’s license or reliable transportation is required.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, or have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred.
