Job summary

Sweetwater Care seeks an LVN with nursing management experience to oversee care within the facility, ensuring compliance with physician orders and state guidelines. The role involves coordinating patient care, education for families, and timely medication administration as per policy.

Qualified candidates will hold an LVN license in California (compact license preferred) and have prior long-term care experience, with familiarity in Point Click Care charting.

Qualifications

• Currently licensed as LVN in California if the contract is in California. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred.
• Long term care experience required.
• Point Click Care charting experience required.

Responsibilities

• Demonstrates knowledge and appropriate use of infection control procedures.
• Participates in Quality Improvement Program implementation.
• Uses Quality Life Manual to identify, place, and monitor patients in programs.
• Notifies other departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely according to facility policy.
• Administers treatments timely according to facility policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications
