Job summary

Sweetwater Care in Oroville, CA is seeking an experienced LVN with nursing management background to lead clinical operations in a long-term care setting. The role requires current California LVN licensure and knowledge of state and federal guidelines.

You will coordinate care, administer medications and treatments per facility policy, support quality improvement efforts, and mentor nursing staff. Familiarity with PCC charting is required.

Qualifications

• Current California LVN license required; compact license acceptable for compact state work.
• One year of nursing management experience preferred.
• Long-term care experience is preferred.
• POINT CLICK CARE charting experience required.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in the Quality Improvement Program implementation.
• Uses the Quality Life Manual for patient placement and monitoring.
• Notifies other departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes and carries out physician orders as permitted by the Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
