Job summary

Sweetwater Care is seeking an experienced LVN nurse manager in California to ensure high-quality patient care in a long-term care setting. The role requires overseeing nursing operations, coordinating with physicians, and guiding staff to meet care standards.

Responsibilities include administering medications and treatments, educating patients and families, and ensuring accurate transcription and execution of physician orders within the Nurse Practice Act.

Qualifications

• Licensed LVN in CA; compact license if working in a compact state.
• 1 year of nursing management experience preferred.
• Long-term care experience.
• Point.Click Care charting experience.

Responsibilities

• Provide nursing care within scope of practice per standards and physician orders.
• Education for patients and families.
• Administer medications and treatments per policy.
• Receive, transcribe and carry out physician orders as allowed by Nurse Practice Act.
• Mimic rounds with physicians as needed.
• Notify departments of patient changes (room moves, admits, discharges).

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
