Sweetwater Care in Santa Rosa, CA is seeking a qualified LVN nurse manager to oversee patient care within the facility's standards and physician orders. You will help implement the quality program and coordinate patient services alongside an interdisciplinary team.

The role requires California LVN licensure (compact license acceptable), and 1+ year of nursing management experience, with a preference for long-term care and familiarity with PointClickCare charting.

Formación

• Licensed LVN in California or compact license.
• Minimum 1 year nursing management experience preferred.
• Long-term care experience required or preferred.
• Experience with PointClickCare charting a plus.

• Participates in Quality Improvement activities and patient education.
• Coordinates patient care, room changes, admits/discharges as needed.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
