Job summary

Sweetwater Care in California is seeking an LVN with prior nursing management experience to oversee clinical care within the facility, ensuring adherence to physician orders and policies.

You will conduct rounds with physicians as needed, administer medications and treatments per policy, and coordinate patient and family education.

The role requires CA licensure as LVN, compact license if working in a compact state, and prior long-term care experience; experience with PointClickCare is a plus.

Qualifications

• Licensed as LVN in California; compact license if working in a compact state.
• One-year nursing management experience preferred.
• Long-term care experience required.
• PointClickCare charting experience preferred.

Responsibilities

• Demonstrates knowledge and appropriate use of infection control procedures.
• Participates in and assists quality improvement programs.
• Identifies and monitors patients in appropriate programs using the Quality Life Manual.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
