Job summary

Sweetwater Care seeks an experienced LVN nurse manager to oversee nursing care within the facility and ensure compliance with standards, physician orders, and policy guidelines.

In this role you will support infection control, participate in quality improvement, educate patients and families, and supervise medication and treatment administration while coordinating rounds with physicians as needed.

Qualifications

• Current LVN license in California.
• Compact license if working in a Compact state.
• One-year nursing management experience preferred.
• Long term care experience.
• Point Click Care charting experience.

Responsibilities

• Demonstrates knowledge and appropriate use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Uses Quality Life Manual for patient placement and monitoring.
• Notifies Departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders.
• M conducts rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
