Qualificações

• BSW is required; MSW preferred.
• State licensure or certification as per state regulations.
• Minimum 1 year healthcare/long-term care experience preferred.

Responsabilidades

• Complete psychosocial assessments at admission and with changes.
• Develop and update social service care plans.
• Provide emotional support and crisis intervention.
• Coordinate with hospitals, home health, hospice, and community agencies.
• Assist with discharge planning and resident education.
• Maintain documentation in the medical record.
