• Experience in skilled nursing facilities is required.
• Strong ability to plan for pre-admission and discharge.
• Ability to assess psychosocial needs and contribute to care plans.
• Advocacy for residents and their rights.

Responsibilities

• Plan for pre-admission and discharge.
• Provide psychosocial assessment and complete MDS sections A, AA, AB, AC, E, F, Q and B where applicable.
• Use MDS to guide the resident care plan.
• Respect and value the worth of each individual.
• Create an atmosphere of growth for the resident.
• Adopt a holistic view of well-being—social, psychological, physical, and spiritual.
• Ensure a supportive physical environment for residents.
• Foster a positive self-image through social contact and independence.
• Participate in resident care planning as part of the interdisciplinary team.
• Counsel residents and families; connect with community resources on the resident's behalf.
• Advocate for and protect residents' rights.
• Ensure social and emotional needs are met.
