Job summary

Outfield Healthcare Partners in Missouri seeks a compassionate Social Services Designee / Social Worker to support residents' psychosocial well-being in a skilled nursing setting.

The position focuses on evaluating social, emotional, and mental health needs, ensuring compliance with NASW Code of Ethics and relevant regulations, and collaborating with the care team to uphold dignity and independence.

Qualifications

• Bachelor's Degree in Social Work or related human services field
• Certified, licensed, or registered in the state of practice
• 1 year of supervised social work experience in a healthcare or geriatric setting
• Skilled Nursing Facility experience preferred

Responsibilities

• Complete social histories and psychosocial assessments
• Accurately complete and utilize MDS sections to guide care planning
• Maintain thorough, timely documentation including care plans, progress notes, behavior tracking, PASARR and discharge planning, advance directives and psych service documentation
• Advocate for resident rights and support self-determination
• Educate residents and families on care options and alternatives
• Collaborate with the interdisciplinary care team to develop and implement individualized care plans
• Monitor and document cognitive, emotional, and psychosocial changes
