Job summary

Atlas Healthcare is seeking a dedicated social worker to support residents through pre-admission and discharge planning. The role includes psycho-social assessments, MDS completion, and guiding care plans with a holistic, person-centered approach.

You will collaborate with an interdisciplinary team to advocate for residents, counsel families, and connect individuals with community resources, while promoting independence and respectful care.

• Psycho-social assessment and completing sections of the minimum data set (A, AA, AB, AC, E, F, Q and sometimes B) using the data to guide the care plan
