Job summary

Blue Point Nursing and Rehab Center is seeking a compassionate licensed social worker (LCSW-C) to join our interdisciplinary team in Maryland. You will support residents and families by addressing psychosocial needs and coordinating care throughout admission, discharge, and transitions.

Key duties include assessments, counseling, discharge planning, and serving as a liaison among residents, families, and care teams.

Qualifications

• Maryland LCSW-C license is required.
• Experience in a skilled nursing facility preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct psychosocial assessments and develop care plans.
• Provide emotional support and counseling to residents and families.
• Coordinate discharge planning and transitions of care.
