Job summary

Complete Care, Inc. in Silver Spring, MD is seeking a licensed Social Worker (LSW) for a full-time day shift in our long-term care setting. The role focuses on psycho-social assessment, discharge planning, and resident education.

Qualifications include an LSW certificate and prior long-term care experience. The position requires independent work, strong documentation, and ability to build rapport with residents and families.

Qualifications

• LSW certificates required.
• Long-Term Care/Skilled Nursing experience in Social Services preferred/required.

Responsibilities

• Identifies cognitive impairments, mood problems and other psycho-social needs in residents.
• Maintain accurate, thorough, and timely documentation.
• Review Advanced Directives and provide education to residents and family members as needed.
• Help provide a smooth admission and discharge process for residents and family members.
• Other duties as assigned by Administrator.
