NALHE is seeking a Medical Social Worker II to assist patients and families in coping with social, emotional, and practical challenges related to illness within hospital and clinic settings. You will provide assessments, referrals, and advocacy to support care coordination with Kaiser and community resources.

• MSW required with licensure as applicable.
• Experience with hospital social work setting preferred.
• Strong interpersonal and communication skills.

Responsabilidades

• Conduct psychosocial assessments and develop individualized plans.
• Provide crisis intervention, grief counseling, and coping strategies.
• Coordinate care and discharge planning with Kaiser and community resources.
• Advise patients and families on Kaiser, Medicare, and Medi-Cal benefits.
• Collaborate with nursing and medical teams to optimize patient care.
• Maintain documentation timely and accurately in electronic records.
